Where can I get an electronic template of a CMS-1500 medical billing form for free?

I work for a non-profit company that provides supported employment services. I only have one client currently on Medicaid Waiver long term support that I have to bill for. I don’t want to pay for software or a Smart Form if I can get one for free. Can anyone make a suggestion? THANKS!

What do I put in box 24E on the CMS 1500 form?

My insurance textbook says to leave it blank, but the software program counts that block wrong if I leave it blank. What am I supposed to put there or do I leave it blank?

Item 24E of the CMS-1500 claim form is a one (1) character field used to point or relate the diagnosis indicated in item 21 to the specific procedure billed. CMS provides specific instructions for completion of this item.

Enter the diagnosis code reference number (1, or 2, or 3, or 4) from item 21 that relates the procedures performed to the primary diagnosis. Enter only one reference number per line item. When multiple services are performed, enter the primary reference number for each service.

If a situation arises where two or more diagnoses are required for a procedure code (e.g., pap smears), you must reference only one of the diagnoses in item 21.

Updating static html page with JS?? (very simple CMS ?)?

Hi Guys,

Building a dinning website with static pages using CSS. There is one page however that will have to be updated once a month by a non-computer literate employee. It just has to show a menu copied from a word doc.

I dont want to use any .php CMS software, just a simple box they paste the menu into which then dumps the text into the correct DIV in the .html page?

Any ideas? could i do this with java-script or some other trickery?

You can do this with AJAX, but I’d strongly encourage the menu be in plain text or HTML, not a word Doc. Word’s formatting is horrible, bloated, and unpredictable, and even the "save as html" feature creates code that will likely cause problems with your existing site.

Use ‘view source’ to see how it works. Note that I’m using the free jQuery library to simplify the AJAX call, and I’ve got several simple HTML stubs.

You might want to provide an online HTML editor for your user to create the stubs (so they think it’s an online form of word but is saved in HTML format.)

Unfortunately, saving the file is one thing you won’t be able to do without a server-side language (because of security limitations) so you’ll need a way to get the file posted with the correct file name. If it’s only once a month, this won’t be too difficult.

why should i put my cms files in public_html?

i am testing different cms softwares & i have always put the files in the root of my domain but a lot of people put the files in public_html folder of the site. . is that Because of security reasons? also, when i put the files in public_html, my index will be there too so when people access my page, the ‘public_html’ link would show up in the address bar which i dont want. how do i get around that? pls only offer serious suggestions UPTO THE POINT. unless you want aextra points on here..

Why is it that my ‘public_html’ appear in the address bar instead of just the domain. i have seen other peoples and theirs dont…. irritating mee

If your hosting service gives you the option to control this in your domain admin, be sure to password protect the directory/directories where you’re storing your CMS apps.

You can also use .htaccess (on LAMP-based hosting services) to control what files are visible.

What backup software would you recommend to backup notebook drive to external USB drive – see added details…

Looking for automated backup software – please consider the following!
- Something that can be set on a schedule.
- Incremental backup after full backup – is it possible to eliminate deleted files folders? i.e. you delete a folder on computer, it is removed from backup? That way I don’t restore and find culmination of unused files/folders.
- Replicates info on hard drive versus using a ‘package’ – some software programs package up backup data and can be restored only with same software. I want to be able to open backup folder from any system (if needed) and have access to files/folders.
- Able to select destination, just just F: drive for example, but F:\Notebook Backup.
- efficient, easy to use interface, reliable, etc!

I have tried CMS software (didn’t remove unused/deleted files on subsequent backups), Windows Vista Backup SW/Retrospect SW by EMC (both ‘packaged up’ backup), and Norton 360 Backup would let me select a destination folder – only to USB Backup Drive.

Cobian Backup. I used it briefly on my work PC, and from that experience, I’m fairly sure it meets all your requirements. Plus, it’s free.

I want to learn one of the better CMS software in the market today. Anyone used Joomla and/or Drupal? It seems those two are the popular one out there. Which one u think it’s better and easier to learn? And why u think that way?

Very heavy traffic, like newspaper, magazine … it is recommended to use drupal.
